To receive a settlement payment, you must submit a properly completed Claim Form so that it is postmarked or otherwise received no later than August 14, 2026. You will need your Class Notice information to get started.

Opting into the Overtime Collective. Your Claim Form lets you elect to join the Overtime Collective under the Fair Labor Standards Act. You will only receive a payment for overtime hours — and release your federal FLSA claims — if you affirmatively select that you are a member of the Overtime Collective on the Claim Form. Returning the Claim Form without that election will still secure your Florida (FMWA) class payment.
Prefer to submit by mail? You may also return your signed Claim Form using the enclosed pre-paid envelope, or by mailing it to the Claims Administrator at ILYM Group, Inc., P.O. Box 2031, Tustin, CA 92781. If you have questions about your ILYM ID or your eligibility, call the Claims Administrator at (888) 250-6810 or e-mail claims@ilymgroup.com. All Claim Forms must be postmarked or otherwise received by August 14, 2026.
